Here's the deal…

I've hooked up with a distributor and I want to run a contest. We've been discussing a few options and the one we like so far is a "soda pop" style contest with unique codes. Once you get your unique code, you'd go to the website, enter your code along with your name, email, and other demographic info. Then we'd pick a grand prize winner along with other follow up winners. 

My problem is that I'm pretty good server side but I'm not fantastic at web programming. I was curious if anyone knew of any PHP or wordpress plugins to meet my needs. I've looked but have came up dry. I imagine it'd be fairly simple. You'd have a list of valid codes and a contact form with a box for the code. Once submitted, the form would check the list for the valid code and if all was well, it'd stay thank you. If not, it'd say sorry, not a valid code or code has been redeemed. 

Ideas?
